I received an email from Manila Immigration and they told me to submit the following:
-original passport of your dependent\s
-2 pieces of passport-sized ID photos
-Completed Appendix A(see attached)

The problem is there's no Appendix A attachment. Is there anyone would help to find the Appendix A PDF online. Thank You in advance.! :)
 
The form can be found here

http://www.cic.gc.ca/english/information/applications/index.asp

and then look for IMM 5669

Print off, fill out and send in :)

I opened the imm 5669 form but it says Schedule A instead of Appendix A. are they the same though ?

Did they give you the number of the form? I do believe that they are the same

I wouldn't assume that it's IMM 5669, and would just reply to the email asking for the proper attachment.

If it turns out to be IMM 5669, so be it, but if you send the wrong form...they might get cranky with you. ;)
 
I doubt that too ! That the IMM 5669 is the same as Schedule A . Please help me if anyone knew the Appendix A form.....

Hi,
IMM5669 is different from appendex A. It is a different form the that comes with passport request from cic.

This came from one of the forum mate. Same as mine.

Hi

The File Number refers to your Application Number. Sometimes they will send this Form without an allocated space for File Number. Just write on the top of the form with your signature.
 
Hey guys, I recently submitted passport for visa stamping. By mistake in Appendix A form I have provided my spouse details whereas she is already in Canada. Will there be any issue? I'm the only one traveling. No family member included in my application.
