1) The wait is usually 3 months for the AOR from the time they receive your application.
2) As far as I know the GCMS notes are usually ordered when you are completely uncertain on what's going on with your application and you've been waiting for over 120 days to hear something from the CIC. But even then, these notes are usually ordered after you get your AOR and 120 days passed after you got it.
