2nd Extension grant letter: April 5 (uploaded via MyCIC the same day)
PPR: April 22

Well, this is my question too but I don't think there is an answer to that. IRCC has rejected some applications after 5 months! :'(
The best thing you could do at this time is ordering GCMS notes to see how processing of your application is going.
