This means you have inactive tuberculosis and would have to be under medical surveillance in canada because this comes under public health danger.
I believe you would get the form at POE, however, you can search for the form on cic website as well.

Thanks for reply. I hv read in other forums about this.

More specifically I am asking, if I have to request via email to visa office to send the form (in particular visa office completes part A) and part B is filled up at PoE or I can go straight away at PoE and both parts can be done together.

I m seeking advice for the best option to have smooth transition. I have still few months to fly.

This week? The week has just started and Tuesday isn't here yet!

Do Ottawa and local offices issue PPRs of the same category in the same week? If that is so, us FSW outlanders will need to keep waiting..

The week starts Sunday for me in my part of the world, lol - Tuesday is already mid-week for me. Honestly, it's hard to say - I guess you could look into the spreadsheet and see if there is a trend, although I would think that given each visa office would have their own targets to meet; it's not necessary that's local VO and Ottawa would be processing and issuing the PPR's for the same category in the same week. Either way, we still have to keep waiting
