Yes you must fill the form, print it, write your name in native language with black pen, sign it and then scan it. Of course you need to use a separate sheet to complete the personal history section. I'm super happy to announce that I received my PPR last night at around 11:00 pm EST!
The email went to my promotions folder, but I caught it since it was titled "Ready for Visa..." and it was from CPCO-FinalRequest. I did not receive an email in mycic account in a matter of fact I checked mycic account about 5 hours before my PPR and there is no change pre and post PPR.
My timeline in the tracker has been updated and can be found in my signature.
