I am in the process of completing my applications so that I can apply within next couple of weeks. I have following questions/concerns about application forms. I know its a long list of questions, but I have tried my best to keep them brief and organized :).....thanks for your time!

Should I include my co-op work experience as well? If I include that I will have to check 2 Years or more, but if I include only my experience after graduation, then I will have to check 1 Year or more.

Starting with your current occupation, list your occupation within the 3 years preceding the date of your application. List only occupations that fall in Skill type 0 or skill levels A or B of the NOC.

So, in this I will only put my experience after graduation (1 year and 1 month), and no co-op experience at all, right?

I am confused because they are asking for work experience within the last 3 years, and I had co-op jobs in the last 3 years, so I am not sure if I should include that or not? please guide me on this?

In the same question they ask for main duties. Does this have to be same as my employment letter or NOC main duties mentioned on HRSDC's website?

Various forms

In various forms they say if there is not enough space to provide all the necessary information, attach to this form a separate sheet of paper with further details.

By this, do they mean a blank paper or that part/page of the actual form? For example, If i need more space for question#8 personal history on Schedule A Background form, should I print out the page with question#8 on it or a blank page is fine?

In my home country primary school is until 5th grade, it starts from nursery (2 year) and after nursery you proceed to grade 1. So, I have 7 years (2 of nursery and 5 of primary) in elementary/primary school, Is that ok?

Secondary school starts from grade 6th to 10, and high school is bascially from grade 11 to 12. So, I have 7 years there as well. Is that ok?

Lastly, I have 5 years for university in Canada, its a 4 year program but with co-op it takes 5 years. So, should I put 4 or 5 years for university section?

In the From and To date, what dates should I put here? Is it the date I first entered in Canada (2006) or most recent entry in Canada (Jan 2012) or the issue date on my work permit (bascially when I got this work permit)?

Guide does not clearly specify what dates to enter. I would assume they are asking for the date you first entered in Canada (in my case its 2006)? Please let me know if you think otherwise.

Q4. No. It does not count as skilled work as it is considered part of an educational program.
Q9. Yes, only experience after graduation. Main duties should be the same as your employment letter.
Various forms - blank page is fine or excel spreadsheet. Name it appropriately so it's clear that it's a continuation of a particular section
Schedule A Background - Q.6d - date refused plus a brief explanation.
Q#7 - Education - a) Yes; b) Yes; c) 4 years. Co-op is considered part of the degree program.
Q#12 - Addresses - begin with your current address and don't leave any gaps.
Q#10 Current country of residence - "from" = date you first entered Canada; "To" = date you fill the form(to be able to validate the form), but when you print it out, cross that out and write "present"
Q#2 Number of years of education in total - from your explanation, nursery is part of primary. In that case, it will be 7+7+4 = 18yrs
Q#2 Have you ever used any other name (e.g Nickname, maiden name, alias, etc)? Yes for your wife. "family name(s)" = her last name before marriage and "given name(s)" = N/A (since she is using the same one from birth)

Answers to your Ques. are:
Q 4 : you can only include after Graduation Work experience i.e 1 year for you. so, select One year or more.QQ
Q 9 :If any of your previous jobs fall in Category 0,A or B only then you need to mention them, if not then only include your after graduation job i.e the job you are doing after your graduation to apply for CEC.
No, Co op experience at all.

please make sure that your duties mentioned in your employment letter should match NOC main duties mentioned on HRSDC's website(Matching doesn't mean to copy & paste, just read those on HRSDC's website and write it on your letter in your own words but the meaning should be same).
In Schedule 8 form also write three duties from your letter only.

I also used extra space for personal history, so the best way is just print other copy of form with Q8 and fill only history section and attach it to main form. And on main Schedule 8 form mention under history section that extra sheet is attached.

#Schedul A - Background/Declaration
Just mention in both forms (you & your wife's) that visitor visa was refused in *****(month & Year ). if possible, mention the reason in 1or2 lines.
Q#7 - Education
Till 12th standard mention according to your home country school system, but for university studies in canada its only 4 years program. Co op is not counted in study. so mention 4 years for your university.
Q#12 - Addresses
Start every question details from recent.(e.g for address, mention first where you are living now and then past ones).

I have one question regarding " Schedule A Background/Declaration " form
the last page indicates

" DO NOT COMPLETE THE FOLLOWING SECTION NOW. YOU MAY BE ASKED TO SIGN IN THE PRESENCE OF A REPRESENTATIVE FO THE CANADIAN GOVERNMENT OR AN OFFICIAL APPOINTED BY THE CANADIAN GOVERNMENT."

does that mean i just sign under declaration of applicant and leave the solemn and interpreter declaration blank?
if i need to fill out this who should i contact and where i can find a Canadian government of an official appointed by Canadian government can sign for me.

Yes, sign the declaration of applicant and leave the "solemn and interpreter declaration" blank. They may or may not ask you to sign it. I left mine blank as well and they didn't ask me to sign it and I got PR status.

For the Generic Application form for canada

Under Application details
#4 Immigration office requested for processing this application
What am I supposed to put here? The immigration offfice in toronto? Or the Philippine consulate in toronto where I got my passport renewed?
Or the immigration office in the Philippines...even though I did not grow up there.
Under personal details
#3 its says UCI.
Can I leave this blank? They said I will not have one with CIC if this is my first application.
What about my application to get my PGWP. That doesnt count?

#9 Memberships or association with organizations

The only thing I put here is College of nurses ontario. What type of organization is this? And what should I put under activities? I only joined it because its what allows me to have a RN lisence to work
So far I just placed "Worked as RN" for activities. Is that sufficient?

RE: Supplementary Info- Your travels

Question # 1: Does it apply to me if my nationality is Hungarian but I was was born is Serbia and after my primary school studies in Serbia I continued with my studies in Hungary...I was back and forth for several years. Which means during the week I was in Hungary and for weekends and went back to Serbia. I spent the holidays in Serbia...That time I was not a Hungarian citizen but I'm a Hungarian citizen today.. (it's complicated)
It say's on the application: " have taken outside your country of origin or of residence" - I'm a little bit confused...

Question # 2: Also coming to Canada counts as a trip?

I have never been in any other country than Hungary-Serbia and Canada in the last 10 years...

Please help!

Is there a particular reason you would like to incclude them? If your employment letter(s) contains all information requested on the checklist, paystubs are just additional. That said, you may still include it. Are you able to get a combined one for each year you worked? You may photocopy them double sided or shrink the size to fit 2 on a page, etc to reduce the amount of paper. :)
