I was assuming a man according to your name ...
For the chart , only name, mailing address, phone and email , no need to sign off. Just first page, name. DOB, POB, ppt number visibility.
I also need to dispatch it on Monday , will take another week to reach there.

So hope for the beset for you and family too!
