You just need to make photocopy and notorize it and English translation and notorize it.
No need to attach the affidavit , as notery public attestation will act as official as true copy.
This is needed for all of your educational documents, marriage certificate, form B for kids under age 18.

When i applied Cio in last year i forgot to attach photocopy of NIC with whole application. Cio didnt return our application. when cio send application to lvo then lvo asked for notarized copy of NIC with additional documents request. My spouse just typed english translation of both NIC and we notarized it from notary public and send them.
 
Which name shows from sender when LVO emailed for additional documents?

Important information. Translations by family members are not acceptable.

Dear All,

Hope everyone is doing well and cases are progressing well. I was expecting to receive PPR1 during September as all other formalities are complete but it hasn't arrived so far :(.

I wanted to know what they ask to submit in PPR1 (passport copies, what else?). If there is some other forms, can someone please share those with me. Basically, I'm leaving for Hajj next week and want to prepare everything so that someone in my family can dispatch while I'm away.
