Any application form for dependent son born in Canada

I am in WP and now applying PR under CEC. My son was born in 2012 in Canada (canadian citizen now). is there any particular form i need to fill for my dependent son. or do i need to attach his pass port / birth certificate and a letter explaining that my son was born in canada and a citizen.
please help me with this question.

Yes, you need to include your Son's details in the family information form. Also, you need to include the copy of the birth certificate and copy of passport ( if you have one). You don't need to add any explanation letter.

Include his details on the IMM 5406 and include a copy of his birth certificate.
