Another Medical Exam Question!

Hi everyone,

We have been lurking here for quite some time, but this is our first time posting. I am a Canadian citizen and am sponsoring my common-law partner, who is from Germany. We are applying inland.

I have a quick question about the medical exam. I wouldn't have been so nervous until I started reading here, and now I'm confused! My partner hard his medical exam done for immigration in September. He took his form IMM 1017 SCL, filled out, with him, and the doctor told him that they will send the form into immigration, and it will be matched up with our application once we send it. They didn't give him any form back to send in.

Now, on the document checklist, it says "Proof of Medical Examination"... but we don't have this form! He tried calling the medical office yesterday (in Victoria, BC), and the receptionist didn't seem to know much, but said that they don't usually give any forms back, and that they send the forms in themselves.

Does this sound right?? Should we be mailing some "proof of examination" in, too? If so, what?

CIC's website makes it seem like the "DMP" will send it.

Another quick question! Thanks in advance for your help, this forum has been a great resource for us! We would be lost without it!

We are not using a representative. Does anyone know if we need to include IMM 5476 (USE OF A REPRESENTATIVE) with N/A written on it, or should we omit that form because it is not relevant?

in the checklist they say: Include for yourself and each of your family members Copy 2 of the Medical Report-Section A that the DMP will sign and give to you when you undergo the medical examination.
were you given this copy by DMP?

no, if you don't use the representative, you just do not sent this form to them at all.

Thanks for your quick reply! The Inland application for Common-Law/Spouse (to Vegreville) only says "Proof of Medical Examination" on Document Checklist (IMM 5443). Is this the same form you were looking at?

I will try calling the DMP's office again tomorrow. We weren't given anything from their office except the receipts of payment. When we asked, they said that they mail it to the office themselves.

I would suggest you call the CIC Call Center and ask what you are supposed to send.

I guess the procedure for inland application is different. The IMM 1017 form is not available to outland applicants, they have to get it from the DMP and it's quite different - for one, it's smaller than the regular format and it's green. I believe Suin was looking at the outland checklist - that one is clearer than the inland one.

the same happened to me so i asked doctor what should i send as a proof of medical examination and the doctor said: the paycheck for medical check up. thats what im gonna do.
