Annex A

Hi Guys,
We have been working on getting sponsorship PR for my wife since July/Aug 2018. We recently got mail from IRCC asking for 1. Passport, 2. Photos 3. Prepaid Envelope and (in their words)
"4. The completed and signed Annex A attached to this correspondence, listing your current mailing address."

Can anyone help me with the "Annex A"? There seems to be No form with that name. Will a cover letter with signature and the current mailing address suffice?

Would appreciate your help if you know or have been through this. Thanks.

No. I have checked and re-checked. The email only has one attachment - primarily Step1 to Step 4 Instructions on how/where to mail the required Documents. But there is nothing else that says Annex A.. no form

It's most likely to be IMM 5669. This is "Annexe A" in French.

Tried but could not get a hold of anyone by phone... I think I'll make a trip to CBSA nearby and see if anyone can explain or have an opinion of how to go about.

I am not sure.. coz this is part of the required forms we sent in almost a year ago (5669)
