The only place that mentions spouse or common-law is on IMM5406 where in the SECTION A, it says to write the name of the principle applicant (first line) and spouse or common-law (second line)....you just need to write the names and fill in the blanks....at the end these two forms need to be signed by the principle applicant!!!!
