If people have waited a year without going in process I would assume that they did not come back to the spreadsheet to update their information. After the Damascus VO closed in 2011 there was a huge backlog in Ankara, so if you are seeing dates from 2011 that could be the reason. Last year in the summer (June-Sept 2013) there was a strike at all Canadian Embassies and again that created delays.
But generally, like Mrs.KOR wrote, usually if a file hasn't moved in process for a long time it could be the officer had to request updated information from the applicant (new forms, additional proofs), the file was incomplete, the medical already expired and/or an interview has been requested.
Some times a visa officer will request an interview for another reason than authenticity of the relationship, but usually not so. In such a case they may call the applicant to clear up any information quickly - this has happened before to other forum users in the past (check an address, occupation, whatever.) But in most cases for Ankara the interview is requested if there are parts of the relationship in question (if you have red flags in your file: large age, education, socio-economic differences; married quickly after meeting; hardly any proof of relationship; no photos; different culture/religion, etc.)
