No you don't need to contact them. AOR2 means the date ndvo(visa office) starts working on your file.
My GCMS note shows exactly same things but no aor 2 since file transferred to NDVO in Jan 2020
Check you ecas if there is a line saying that they started working on your application after it got transferred to ndvo

i applied end of feb 2019 ... April 13, 2019 medical approved and passed, june 13, 2019 biometrics done .. filed transferred in June of 2019 to new delhi ... september 28, 2019 they gave us prearrival services letter and since then waiting as csq was only submitted by quebec govt in december and i had applied in July for it ...
Usually medical request comes after you showed proof to NDVO that you have applied for CSQ because MIFI is delaying CSQ's.
My timeline:
Spousal sponsorship QUEBEC
Application received CPC Sydney: July 24
AOR1: Sept. 18
Biometrics: Sept. 17
Biometrics completed: Sept. 19
File transferred to NDVO and CSQ invitation: Nov. 5
CSQ application sent: Nov. 9
CSQ application received by MIFI: Nov. 12
NDVO processing: Nov. 22
CSQ AOR: Dec. 2
CSQ request by NDVO: Feb. 5
(given 45 days to submit)
Sent CSQ AOR copy to NDVO: Feb. 5
Medical Request: Feb. 6
Medical passed: Feb. 21
CSQ approved: May 25
CSQ sent by MIFI: May 28
CSQ received by mail: June 2
CSQ copy sent to NDVO: June 2

Well no actually they had asked for medical before and only got transferred in July of 2019 ...and only asked us to submit csq july 19, 2019 ..
I checked my ecas. Medical received on 9 jan. There is no line like we start processing your application
I guess your medical has to be redone because it expired. I hope NDVO will exempt it.
Yah i have a feeling itll have to be redone .. i hope theres an exception and they let it go cause genuinely not our fault covid-19 happened ..
