the form is not available on internet as it comes with your name and application number printed on it.

2) Is the Photo is "Passport size" or "As per previous specification given in CIC website"
As per CIC website. But in my case IOM took passport photos as I didn't had those photos with me at that moment.

3) Do I need any prior appointment to Medical Center? How long it takes?
Yes you need to call them. the procedure is only about 30 minutes. But if there are many people it will take longer.
4) How much money do I need (if you done recently)?

In addition, I have found here previously (Page 23~24, Posted by "Tomsware") that after medical, need to email to CHC? Is it a regular case OR It will be mentioned in the email whether I have to make email reply to CHC??... Can you kindly clarify.

It will be mentioned clearly in your email. I think now it is regular to email them after completing medicals as reedoy has done it so.

Hi all,

After I completed my undergrad in UK in 2011 I applied for a post study work permit there which is valid till Oct-2013. After completeing my studies in UK I did a couple of internships each lasting 3-4 months and then started working in October'12 on a freelance basis for a small company based in UK. I left UK in November 2012 and have been staying in Bangladesh since. I am still working for that UK employer on a freelance basis. They only pay me on project basis and does not require me to pay any tax. I only work when they send me task not all the time.

My question is do I still need to include any employment related papers or tax stuff with my visa applicaton? It wouldn't make any sense anyway because all I did was a couple of internships in UK and now part-time on a freelance basis. These are not proper jobs and I don't have any papers from the employer as well. Also I am currently residing in Bangladesh. What do you guys say?

P.S. Having said that I am mentioning about my internships and freelance job in the application form. I also have to submit a CV, as it mentions that anyone who has been out of education for more than 2 years should include a CV. Please let me know, I would really appreciate

At first, showing employment is not mandatory. But I like to say, freelancing is a well established self-dependent business which is appreciated all over the world, although there r some misconception among BD people. So, better u mention freelance as current profession in ur CV and set a short description about it. U can show ur bank statement where u have ur foreign currency transaction by which u can prove ur monthly earning. Mention that ur earning is tax free. I think that will be enough.
 
Thanks Rasel,well I am already providing a CV mentioning it's freelance and part-time, and also putting in the application form just for the sake of it so that it matches with the CV.Morevover since I am not sponsoring mysef I rather not complicate things by including the tax stuffs and bank statement of foreign transactions. I hope the visa Visa officier has the ability to understand that these are casual jobs without any proper papers as I am only a recent graduate.

Anyways there is another issue I would like to discuss now. As my father is sponsoring me, someof his business documents are in bangla, like the tax certificates and stuffs. Does those need to be translated?

If I need my passport urgently, which I have submitted after completing my medical test, what should I do?
 
you cannot do anything. you can request but giving is totally up-to them.
 
Then I am in serious trouble.!!! Can VFS office help me regarding this?
 
I do not think so. Suppose you are able to get it back it would take like two- three weeks. One week for replying to your query and another for getting back.
