If the company is changed internally then you wont require to know them.
If the company name changed and you transferred from the Old company to New company then you must have a transfer letter. If the office address has changed then send the Transfer letter, a clarification letter from HR or Director and the new office address. Also give a forwarding letter if u wont get clarification letter from HR or Director. Give all docs with old company pad.
