Well, if that happened definitely you should provide all of those information about related on Application, UCI #/ File#. Cause of interview and result outcome of that interview.
Remember you can't hide any information, rather good to let them know your honesty and they will feel your intension.
Please prepare a cover letter and send in both ways by raise CSE and courier..
