Hi all,

If anyone hasn't got the above form from the CHC while receiving their PRVs+COPR, and have kid/s under 16, plz email me on
muhammad.saeed.cvf@gmail.com , I will send them.

U'd need this form to be filled, signed & stamped by your doctor, from your homeland, before departure for landing. It's required for the kid's School Admission in Canada.

And that's a strictly enforced regulation in Canada.

I got medical form today in a separate e-mail for all. While going for Medical shall I take immunization form from the same Doctor for my child?

Hi everyone,

I am submitting two declaration letters for proving the PROOF OF RELATIONSHIP in ( Adaptibility factor), as the birth certificate of my Canadian Uncle is not available.

1) One declaration is given by my Canadian uncle , stating that I m his biological nephew and my father is his biological brother.

2) Second declaration is given by my father, stating that I am his biological son and my Canadian uncle is his biological brother.

Both the letters are attested by the foreign ministry , notarized as well and done on the stamp paper.

Now my question is that , Do I also need to a third write a declaration letter stating that I'm a biological nephew of my Canadian uncle and biological son of my father ? or Shall I write a cover letter and explain the whole situation that these were the factors that led me to arrange the declaration letters ?

you should submit birth certificate for your father & uncle that they are brothers from same parents, also your birth certifcate to prove you are son of your father.

My wife lives in Ottawa and I am the main applicant in Dhaka. Is that alright if she send her passport to me to send it to SGVO when they will ask for PP or she needs to be here to send the PP? Really confused!

Could anyone please advice me about it.

I have not seen any form named IMM8...

Just for ur info...for child addition just follow the below docs ....they will add accordingly....This is already followed by several applicants ....


1. Updated Additional Family Information IMM5406” FOR U AND SPOUSE.
2. 150 CAD bank draft
3. With a cover letter mentioning your new born child addition.
4. Child Birth Certificate
5. Passport.
6. updated Bank cert (+statement )
7. PHOTO-6 COPIES (WITH FIVE COPIES WRITTEN WITH NAME+DOB+Photo taken date )
8. Generic Application form

Bhaiya thanks for the info. Should I send the bank certificates along with the statements? Most of the money is in FDR only little is in savings account. Previously I sent only bank certificates. What's your opinion. If statement then for how many months? 3 months will be OK?

My NOC Code is 6211. I submitted my RPRF at 23rd Oct. and they respond me with payment receipt. Can u tell me what is the next step? and actually how long it take to have a response from them?

Yes this is absolutely acceptable....Many applicants from last year did the same thing....Many Indian applicant sent their PP via their friend/relative to India then to NDVO....There were no problem...

U can bring her PP with an authorization letter...
