U have already told that SGVO has sent all communication with ur corrected name. Can u please confirm what name was mentioned on ur last issued attached Medical form?

Also what they mention on the emails that u received? Like

" Dear XXX YYY ZZZ"

If this Name is in corrected order (First, Middle and Last name) and If attached Medical request form is in corrected form than in my opinion they already corrected ur name as per ur passssport. In this case u r safe. Then u might not need any further action.. :)

Put N/A or NONE. Don't write 'Blank'. Case officer may think you have forgotten to fill that space.

"Not Applicable" , N/A, NONE everything is okay. But first one is better one in my sense.. :)

Brother my questing is that, Am I write “Not Applicable” in my hand by pen ? or is there any option to select “Not Applicable” in that form ??

Sorry brother, For generic form when u kept blank it will be inactivate (will be ash colored). So no option for writing in hand... :).

So keep them blank and press 'Validate' button after filling up the form.

Write down in short form like Dhaka-Dhk, ...This is acceptable since they did not give u scope to write down in full form.

Today I send to Singapore following documents as per visa office emailed requirement.
1. Medical done to Mr. Wahab
2. Police clearance certificate
3. 04 pcs photo
4. visa fee CAD $980.00 (including my wife)
5. Net worth statement with bank evidence.

Requesting everybody generally how any day need to finalize my application???

Need an urgent help from the seniors. I went to SCB Gulshan branch today to issue a bank draft of 980 CAD as RPRF. But the bank staff refused to issue me the bank draft as he was asking me for a fee schedule form where our names and address should be written on it. But I have no such forms with me and did not find any such form on their website as well. Can anybody please help me out in this regard?

SVO email you where mention to pay the visa fee for you & your spouse CAD $ 980.00 .
SCB want that email for issue bank draft you need also following papers for issue Bank draft
1. Application letter
2. SVO email copy
3. Marriage certificate.
4. Passport copy.

How do i will charge my laptop or mobile which are compliant with 220V? Is there any voltage convertor we can use to charge these devices?
If yes, where can we get it?

Please give some lights regarding this mater.

Thank you all forum mates especially Mute_man & Ashif_eee for your valuable time and kind call during my torturous moments. As there is no form called fee schedule form on cic website I personally talked with the manager and complained about the arrogant and misinformed colleague of him and also explained him the matter. He then asked his subordinate to issue me the bank draft according to the email that I received from London Visa Office. Later on, the colleague issued me the bank draft. What a hectic day for me . Now the wait of 48 hours to see whether we passed our medical or if there is any problem. Again a big thank you to all of you.
