Please use the latest forms found on CIC website.

The following documents should be submitted in order to add your child to your application:
- processing fee for the child in the form of a bank draft for C$150 payable to "Receiver General for Canada"
- updated/latest IMM0008 Generic and updated IMM5406 “Additional Family Information” form for yourself and your spouse
- notarized copy of the child’s birth certificate
- copy of your child’s valid passport
- four recent passport-size photographs of your child according to the specifications

if u r old passport has enough time on it , advice u not to change it.
but if u r mad about to change it , then change it to machine readable and send it with a cover letter explaining the fact. but why u want to take the un necessary hassle. please tell us the reason , so that we can do the same, like u :

MRP is not mandetory for immigration to Canada, but I think, it will take another 3/4 years to have canadian passport for all FSW Immigrants. Right now, Bahrain does not issue Visa if Bangladeshi Hand Written Passports are produced. However, MRP is Ok for them. In refusal letter they specifically mentioned about it. So, there are strong chances that, in the mean time, a lot of other countries will also follow Bahrain. Even Bangladesh government may decide not to allow passports other than MRP. (Majhe Kintu Ei Rokom Ekta Kotha Shuna Giyechilo.) Ar Amar to Shara duniya Travel Korar Ichcha ache . Shei jonnyo ami kono chance nite chai na...

Definitely it is an individualistic decision whether to go for conversion to MRP or not.
