my aunt has a problem, she uses her husbands sur name after her name, her name is 'Shaila'.she uses 'Shaila Rahman' now, in her national ID card,birth ID,all the bank account and her children certificates her name is 'shaila Rahman'.As her original name was Shaila Khan is it needed to correct the name from all documents? especially from her Birth ID card?

It is common in Canada, Europe or USA with women names. They have birth name and then changed name. Last month when I went to renew my USA driving license, the lady before me came to change her name adding her husband`s name.
Do not worry, she has to submit all the document including marriage certificate(Nikahnama) and an affidavit that after her marriage she had taken new name.

my medical form has an extra sapce inserted inbetween the name, like 'SANA ULLAH' instead of 'SANAULLAH'...the spelling is otherwise correct. Should I go ahead with this form?

Birth ID = Shaila Rahman (with husband's name,previously she has no birth certificate it was taken at 2009)
National ID = Shaila Rahman (she got NID at 2009)
Her own certificates = Shaila Khan (her name family name)
Her childrens certificates = Shaila Rahman
Her bank account and insurance name = Shaila Rahman
Her passport = Shaila Rahman (It is very near to expiry,so it can easily change)

Its a problem indeed. Sana Ullah and Sanaullah are not the same. Throw a query to visa office about this difference immediately, they'll reply. Don't forget to write "Urgent" before the subject.

Your Aunt is supposed to have an Affidavit for changing her name. Since, the Nikah/Marriage registry requires a document as a proof of bride's name, and the registrar see the maiden name (Name with father's or family surname) in the SSC or any other certificte, he requires an Affidavit of changing her name. Upon getting the Affidavit copy of change of name "Halaf Nama" the Nikah registrar enters the name of bride and bridegroom in his registrar and issue Nikahnama or Marriage certificate.

You are in need of that Affidavit copy of your Aunt's Halafnama.

1. She will have to rectify Birth Certificate.
2. Do accordingly instructed by Indianajones, this is a very good suggestion and good explanation. Don'[t forget to give him a +1 ............I have given him already ;D
3. After she was married, name (Shaila Rahman) in all other documents is 100% logical, hopefully they were opened after her marriage.
