You must use the new forms... & so did I...
Below is the list of docs I sent.....
1. IMM0008 – Generic – Print, sign, and date on page 8.
2. IMM5669 – Schedule A – Print and write your full name in your native script on page 1, beside question #2, and sign and date
on page 4 under Declaration of Applicant only.
3. IMM5669 – Schedule A – Addendum (If needed by you)– No signature required on this form.
4. IMM0008 – Schedule 3 – Print, sign and date on page 2.
5. IMM5406 – Additional Family Info – Print, sign and date under Section D only.
6. 6 new photographs as per canada visa instructions for all family members;
7. Original financial documents (e.g. bank statements, term deposit statements, property documents with valuation reports,
Insurance policy docs, Share investment) for the past 6 months showing transactions for all accounts as proof of unencumbered
funds for prospective settlement in Canada – any large deposits must have clearly documented provenance to show the funds
belong to you.
8. Copies of your national identity documents with certified English translation.
if u r accompanying your spouse then u will have to send the below forms as well for him/her...
