Thanks all for cordial response. I previously mentioned that my MR package was missed/lost. However later they (SGVO) have sent it through e-mail and asked me to submit PCC, Photos, RPRF and passport also. But they have supplied only Appendix A & C. No attached instruction for PP and RPRF. Could any senior members who already passed the stage provide me the necessary information as well as the necessary instruction and/or forms.

It's not only MR, it's MR and PPR!! That's great.
As far I remember, appendix A is the form for your passport information like: first name, family name, height, eye color, nationality, passport expiry date etc. Isn't it? Then it is the form for passport submission.
1. Apply for PCC in DMP office, Baily road. Instructions are in their website. It will take exactly 7 days. You have to do a challan of 500 tk for every person.
2. Go for the medical as per instuctions. Preserve the money receipt they give you. The next day, ask for the tracking no. of DHL/FEDEX.

4. There is no separate form for RPRF. For bank draft, go to Gulshan SCB with money, medical receipt copy, IMM 5669 copy for every person, Nikahnama photocopy (if u r married) and the passport photocopy.Sometimes they want to see the originals. So,keep them with you.
5. Fill up IMM 5669, appendix A and C, collect all the documents and then after 2 weeks of doing medical, submit them through VFS. They will charge 4150 tk per passport. Or you can go in person if you want.
