It should not be IMM5669-the background declaration form. SGVO asks for both IMM5669 and Appendix A.As far I know it is not available in the website and it should contain the barcode sticker.

I cannot attach the image here, otherwise I can show the scanned copy of Appendix A.

This appendix A is a table requiring some of your information you need to tell them along with submitting the passports like- given name, family name, eye color, height, date of birth, place of birth, PP no. , Citizenship etc.

And another point of the form is - how you are going to send the PP- in person/courier/VFS

Yes you r right. IMM5669 is not Appendix-A. This form is used as a proof copy of your information which will be used to fill the visa sticker. They are asking it only to confirm the information by us. They scan the barcode to open our file which makes the process faster and reduce their workload.

By the grace of Almighty Allah, just now I have got Passport Request.

Be noted that I sent an email to SGVO yesterday at re-singapore-im-enquiry @ international.gc.ca from my mail ID & got a reply today a few minutes ago along with PPR. I also sent them a CSE on 05.01.2013 but they did not reply.

Now I am just waiting for my daughter's MRP Passport which will be delivered to me on 14.01.2013. I will submit our passports on the following day.

Thank you all ! Hope PPR news of everyone will come soon in the coming days ! Have faith in Allah !

No you should not. It will be too early. You should wait atleast 60-70 days to do so as they r now working on your file.
