If I were you I wouldn't reply to that email. Check the authenticity of the email!
Any e-mails sent to you by CIC will end in “@cic.gc.ca” or “@international.gc.ca.”

seems like just the tazkira and the questionnaire, not the whole thing!!
don't worry, send it. I think it is the IMM 3911 if im not mistaken?
 
@Kat: Agree with Hope. It's not the entire application, just the supplementary forms for Afghan nationals. You can just copy/paste from the original application, unless there have been changes in travel, education, or job history.

Yeah, your husband has to do the questionnaires and send a copy of his Tazkira ID card.

I THINK you have to send it via mail and not email (through mail you could get the tracking number).

no its not, as long as you have written permission from that person to have their passport in your possession it is not illegal.

Please let me know how did you make tazkira for your self, have you provided them tazkira of your family member or how , what docuemnts did they asked from you in order to issue you Tazkira? please reply me. Thanks brother

Scenerio 1: If your husband did not complete the form on the link that cic attached in the email, then he should read every page and complete it with your help.

Scenerio 2: If your husband completed all the forms in that link except the supplementary forms:

You are asked to submit the supplementary forms. Did you complete every form on Page A7 and A8?

The supplementary forms include travel history, education and employment history, and military service. These are found on page 12 under "supplementary information form". It is 4 pages.

What I understand is that your husband completed everything on that link except the supplementary forms and he did not send his tazkira identity card.

oh i think the afghan national questionnaire is in french i dont know why :S It's like the second half of the pdf file. very confused

I have the PDF file from the link which i downloaded 2 years ago and it's in ENGLISH (including the other half). It looks the same but there might be small changes. if you want i could send it to your email but you HAVE to check if there any changes. If there are changes, don't use it.

Another option is that i believe you could get this form from the CIC resource center near your city.
