Address on medical form is wrong

Hi all. recently I received request for medical through form IMM1017. However the name, UCI and date of birth mentioned on the form is that of my daughter who is my non-accompanying dependent and lives in Philippines while the address mentioned on the form is mine (I live in Canada). Did anyone else face this issue? and what did you do about it?
