Address for PR card renewal

I am in the process of renewing filling the form for PR renewal, and need clarification on what I should do regarding question 21.

1) The period to be assessed, I assume I put the date I landed as the from date, but what should the end date be, the date I send the app or the expiry date of the PR Card?

2) My husband and I moved a lot for his work projects, in the last year or so, we decided not to rent a place. Do I list these temp residences as my address? My husband used our last rental address and had our mail forwarded, he suggested I use that address to cover the time we lived in temp housing I am not sure I comfortable with this, what do you all suggest I do? Thanks.

For:
1) Depends on when did u land. It should be the last 5 years or since you landed. If you are applying in May 20, 2017 then count 5 years back that is May 20, 2012. If you landed after May 20, 2012 then you should write your landed date.

2) You should write all the addresses. For myself, I sent the tax statements which show my current address. They are going to ask you for supporting documents, such as tax statements, copy of your passports and entry/exit to any other countries you traveled to
