Address for non accompanying dependant on medical request

Hi everyone

So I finally got my request for medicals, I have already done my own and they have also sent a request for my daughter who is not with me in Canada (I expected this)

My question is, they have put my address in her medical request instead of her address, is this normal or should her address be in there instead of mine?

Have you informed them about her address (in any forms)?

Yes it's on the form that I list family members on, her address is on that form.

Is there a better way to contact them rather than playing the CIC hotline lottery?
