Additional Information Form

Hi Guys,

Just wondering,

I am being sponsored by my common-law partner. I am attempting to fill out my additional family information form ( IMM 5406)

do i include him as my common-law partner on this form or not? it doesnt really give a clear answer on the website.

Hey OZinOn,

if you're talking about the section A, yes, include your common-law partner ( who is sponsoring you).
At least that's what I did, and it worked..

apparently i am unable to send inbox messages?

I have another question about this form. it says in the instruction guide that i must answer all questions and if not applicable to me write N/A in the box.

I have entered all the appropriate information in section A (myself, common-law partner/sponsor, mother and father.

then i signed the bottom of section B as i dont have any children, but it says for this section i must answer all questions? do i need to write N/A in all the boxes under this section?

also i only have 2 brothers so do i need to write N/A in the empty boxes underneath? i do wish that these guides were even a little more specific!

oh and it says that my common-law partner needs to fill out this form also but i do not see it under the sponsorship heading where we gather all the forms?

I forgot about that - you can not send PM until you have reached 10 posts, so you're almost there! 3 more to go, and you'll be able to add information under your profile, and send private messages to people on the forum.
Yes - put N/A whenever it doesn't apply to you. It's especially important with children because if you have children and do not declare them here ( and have them medically examined), you can NEVER sponsor them later on. It might not apply to you, so no worries, but CIC wants to make sure that you are aware of what you're declaring. So fill out the form, and put N/A whenever needed.

For section C - list your 2 brothers, and that's it - no need to put N/A, that's only if you are not filling out any information at all - just to make clear to CIC that you didn't fill out this part because it doesn't apply to you, not because you didn't see it.

I don't remember that my sponsor ( also common-law partner) had to fill it one at all, but I'll check and come back to you on that one.
