I am filling form of additional family information. I think me and my wife both have to fill out this form. Am I right?

when my wife fillls the form.,,, she should mention herself as applicant and me as spose.... please confirm?

Also my wife's mother is no more living, but there is no space to mention about her date of decesed... how to mention this fact and what to write in present address colum

one more thing.. form also mention to write name in Native language. My native language is Gujarati... shall I write name in Gujarati on same form ( just below the name written in english) or I have to fill complete new form with all information ( including address) written in Gujarati ( native language)

In Maritus Status for your mother you write Deceased and on the address you write N/A.
