Hi Everyone please help !!

One question about additional family information form

Its says completed , signed by all the members in this following list, so I believe there will be two forms, One I must fill and sign and another my wife must fill and sign
So when my wife is going to fill that form under section A, is she going to put her name beside applicant and i will be beside spouse section or vice versa.??

You got it right. When you fill the form, you will be the applicant and she your spouse.
Another copy, she fills as the applicant and your info to be mentioned as the spouse.

Andy, what is the form # for Spouse and do I need to fill a similar for my kid who is 5yrs

In your case, only you and your spouse need to fill individually. Not your kid.
