ADDITIONAL FAMILY INFORMATION - IMM 5406 (02-2018)

Hi,

I am applying under Spouse ( Including Dependent Children) . I have following questions regarding new format of IMM 5406 - ( 02-2018)

1. My daughter is born in Sultanate of Oman but she is a Canadian Citizen as I am a Canadian Citizen.
Can Any one can please guide me the definition of country of birth?

As per form we need to mention country of Birth. In my daughter's case is it Oman or Canada?

2. Also the New form has less rows for listing brothers and sisters . However it mentions, attach separate sheet. Does this mean on a separate sheet of paper or we have to use one more additional IMM5406 form as additional sheet ?

3. What are the changes and the new forms under Spouse ( including Dependent children ) as I have come across the new versions of :

IMM 5406 - ( 02-2018)
IMM 1344 ( 02-2018)
IMM 0008 ( 10-2017)

Appreciate if some one can help me with the answers for the above questions.

Hi

1. They aren't asking about citizenship at birth and she was not born in Canada.

2. Either would be accepted. If it is going to take more than one extra copy for the additional info, I would use a separate sheet.
