Hey!!

Well this form is a head melter!! I am the applicant and my Canadian wife is sponsoring me. I have one dependent child but she will not accompany me.

Ok so "Section A" I fill in me being the applicant and not the sponsor in the Applicant section. The next section is confusing though I am married (obviously) to my canadian wife so do I put her name in here? But then "Note 1" what do I do?? Do I sign this section to certify I am not married to anyone else or what??

Do I put my Mother and Fathers details in here even though they are not accompanying me??

"Section B" I have a daugther who will not be accompanying me. So do I fill her info in her but do I sign "Note 2"??

"Section C" I have one brother so I just put his info in here even though he will not accompany me??

Section A - you going to write "your" name under Applicant and "your wife's" name under sponsor.
And just leave the signing part for "section A" BLANK, you don't have to sign that as you are married.

Section B- Yes, write your daugthers name and don't sign, as you do have one right.
Section C- Your brother /sister (even though if they are not accompanying you to Canada).

P.S : YOU WILL NEED TO ONLY THE SIGN THE SECTION D okies.....

Ummmm I didn't wrote anything i just left it blank !! But anyway that shouldn't matter if you do/don't because you are providing the name on those sections above, so i guess that should be alright.

Your spouse, mother, father, brothers, sisters and your children, even if they are (or are not) immigrating to Canada. Even if they are already citizens.
