Hello,

In ADDITIONAL FAMILY INFORMATION application form which also has to be filled by spouse .
Must she indicate her name into APPLICANT field or Spouse filed?

If she is the primary applicant then in applicant and her husband name in Spouse if she is secondary applicant then husband name in applicant and her name in spouse.

Look, EG7 instruction states that:
Additional family information (IMM 5406) form must be completed by:
You, as the principal applicant;
Your spouse or common-law partner

So it does mean that i have to send 2 application one filled by me as principal applicant and second one filled by my spouse as spouse or common law partner.
But this form doesn't have any option indicating to whom does it support (form SCHEDULE 1 also have to be completed by spouse, but this application have on the beginning the option indicating to whom this application belongs.

It seems very confusing.

Yes you will fill the both forms and you will put your name in the applicant when you will fill and your spouse will put her name in the spouse section for your form. And when your spouse is filling that form she will put her name in the Applicant and you will put your name in Spouse. CIC will know from the Applicants name that which form belongs to whom.

Hi everyone,
This form confuses me as well. I am being sponsored by my wife who lives in Canada. I am moving there.
When I fill out this form do I include my mother and father and brothers? They live in the states and are not moving. Or do I just put down my wife's information and put N/A on the rest?
Also, does my wife have to fill out an application? And what should she include? Me, Her Mother and Father and brothers? Or just me?
