additional family form

Hi everyone. I just want to seek advice on what to do if i do not have any info on my father for 10 years now and the form is asking for his address, what should i write? And if my father has children on his previous marriage that i know nothing about, what should i tell? Lastly what what would be the marital status of my parents if they were never married and got separated, without all the legal papers to support this?
I appreciate all the help i can get...

A similar situation happened to me. I entered "N/A - info not available" then in a separate blank paper, I wrote that I haven't had contact with my dad for more than 15 years(in my case) and I have heard from other members of the family that he now has another family with kids that I don't know anything about. I also stated that my parents never got married so my mom's marital status is "single."
My application was accepted without asking further details.
