Reference form no IMM008_1e ( back ground deceleration) it is clearly mentioned on the form under instruction and as well on document checklist ( Form No IMM5612E) that it will be filled separately by the principle applicant, spouse and any children above 18 years of age. There also a check box on the form that clearly defines that who fill the form.
But on form no IMM5406E no such instructions and check boxes are given on the form. But on the document checklist ( Form no IMM 5612E) it is mentioned that it will be filled separately by principle applicant, spouse and any children above 18 years.

I will be obliged if any one could clarify that either I have to fill form no IMM5406E this for myself only or my spouse and children will also have to fill this form.

You and your spouse have to fill separate IMM5406E forms.
 
IMM 5406 has to be completed by you and your spouse.
But make sure for your spouse form put your name in applicant field, your spouse name in spouse field and your spouse's family info in mother, father, brother & sister field.

But still i have question why not it is instructed on the form as clear instructions are on back ground deceleration form.

No.... When filling up of spouse's form, we need to put spouse's name against "applicant" field and principal applicant name against "spouse" field.
And, give detail of the family of the person whose name written against "applicant" field.
Of course the detail of spouse's family.

Sorry didn't get your question. However, If you are asking to write names of family members than write the names of all your family members in your form and all family members' name of your spouse in her/his form.

Write name (only name) in both ienglish and in native language.

Yes, I just did the same thing.
Kept my name as the applicant in my wife's form and I got my AOR from Sydney.

So you put your name in both form as a principal applicant.... then got AOR... I did the same thing... I was relieved.

Is it confirm that while filling out from IMM5406E for your spouse, your name will still be in applicant field? Do you have to mark that form that it is spouse's?

No. In your spouse form, she should be the applicant. Its confirmed by seniors.
