We applied for PR through EE and were selected through category based immigration under healthcare. We applied for PR and even got Medical request and Biometric, but my wife could not go for medical since she was expecting. We still went for medicals and informed the doctor and they said they will wait for the Xray until after the delivery. After the delivery we sent a request through webform for addition.
Now they have sent an email requesting to submit "Child add on kit" and on the application they have asked for:

We are not sure what to put on the first one nor what do they mean by Kit, and which application forms are required.
Can anyone help please as we don't have much time.
