Adding newborn form imm5406

Hi All,

I am blessed with a baby. My husband is primary applicant and our application is still in progress. we received notification to fill IMM5406 form to add the newborn. I am confused how to fill the IMM5406 form. Whose name should I fill in the applicant name under Section A. Should it be my husband name or my baby name.

Also if it is husband name, Do I also need to fill the same form, means total 2 form ; one by my husband and one for me.
