My husband, Canadian citizen, is sponsoring me, a US citizen, outland application. We have no kids. My husband has been in two common-law relationships in the past in Canada. All of the girls have been Canadian.

We got the forms for my husband (my sponsor):
Application to Sponsor IMM 1344
Sponsorship Evaluation IMM 5481
Additional Family Information IMM 5406 (for his additional family info)
Receipt IMM 5401 (that we need to still get paid and printed)
Sponsor Questionnaire IMM 5540

Now for supporting documents for him we have:
Canadian birth certificate
Canadian passport
Marriage certificate (should that go with my documents or his pile?)
2 letters of severance (for both of his common-law relationships cause I heard the application can be returned if we don't supply those) - we are going to get those tomorrow
his T4 as "Option C Printout" (is that correct?)
letter from employer.

For me (sponsored spouse) we have forms:
Generic Application Form for Canada IMM0008
Schedule A- Background/Declaration IMM5669
Additional Family Information IMM5406 (I have one and my husband made one, should we include both together in my portion?)
Spouse/Partner Questionnaire IMM5490
Application for Permanent Resident Card IMM 5444 and Supplementary Identification Form IMM5455

We also have my birth certificate (translated and certified since I was born in Poland), Copy copy of my extension visa (it couldn't hurt could it?),travel documents (aka my passport page), FBI clearance, Medical Exam...

Of course we got tons of other proof that probably killed a tree or two...that is going in it's own envelope...

For the forms and documents, am I missing anything??? Thanks a bunch ;D

i don't believe a t4 can be used as a substitute for an option c form. It says in the application that if an option c form is not available, then an explanation is needed and to submit 12 months worth of paystubs. i don't think they just mean the t4. perhaps a senior poster can tell you for sure. is there a reason you aren't sending the option c form? if he's done his taxes, he will be able to receive one from CRA, just by calling. It's quite easy to do. This is what CIC uses to make sure the sponsor hasn't used social assistance. i don't think a t4 will prove that... i'm not sure it provides enough information.

your husband does not need to fill out an additional family form. this is only for the applicant, and what cic uses to determine future sponsorship ability. your husband is canadian, so it's assumed so are his parents, children (if any) and siblings. They will not need to be sponsored in the future.

i also don't think your husband needs to send his birth certificate. only 1 identity document is needed, and that's his passport.

if you are adding these items because it says to include it for the spouse of the applicant, they are not referring to the sponsor. The spouse they are referring to is if both you and your spouse are being sponsored for PR by another person, NOT the spouse that is the sponsor.

it's important to look at the checklists provided to be sure you are sending the proper documents. there is 1 checklist for sponsors, 1 checklist for applicants, and there is a checklist that combines both.

My husband assumed that a T4 is enough... I don't know anything about Canadian forms, so I didn't question it. Again, I wouldn't be able to tell the difference between a t4 or Option C Form.

Ok.. so passport only.. got it.

"if you are adding these items because it says to include it for the spouse of the applicant, they are not referring to the sponsor. "it's important to look at the checklists provided to be sure you are sending the proper documents. there is 1 checklist for sponsors, 1 checklist for applicants, and there is a checklist that combines both. " I am doing that. As you can maybe tell, I went down the list according to the guidelines provided. I included the copy of extension and his birth certificate as extra proof of identity.. also I included the application for PR because I got advice on here that it'd good to send it with the application as it will speed things up.

A T4 is not enough. A NoA (Notice of Assessment) letter from CRA is better. If you have filed taxes, you will get an NoA for that year. An NoA is more similar to an Option C Form.

However, an NoA is not an Option C. No guarantees that an NoA is enough. By far an Option C is your best chance for speedy processing.

a T4 is not the same thing as an option c form. A t4 is what we call a w2. It will not provide the information CIC is looking for.
i don't think we have an equivalent to an option c form. it's basically a detailed notice of assessment from the Canada Revenue Agency. He can get it for any tax year he's done. So, he needs to call CRA NOW to request it. It can take 10-14 days to receive it in the mail, and CIC requires the original document. The printout available online will not count. If he hasn't done taxes, he needs to do that ASAP because it can take 4 weeks for CRA to finalize the return and make an Option C available. They you would have to wait to receive it in the mail. If he can find 12 months of paystubs, then that's the other alternative CIC accepts. I dont think a t4 says when payments started, only a year to date breakdown.

I just wasn't sure why you were sending an additional family form and birth certificate for your husband. some people get confused on the wording on the application. so only an additional family form and birth certificate is necessary for the APPLICANT, not the SPONSOR.

i've never heard that sending the the application for pr card will speed things up. it really has no bearing on the application. perhaps it just eliminates a step after receiving PPR? I don't think the form is completed until landing, though i can be wrong. There's also no guarantee sending that form now will stay with your application for the entire process. Also, CIC regulary updates forms, so there's no guarantee you won't have to fill out another form later on. I guess it really doesn't matter either way.

CIC will not need extra proof of identity for your husband, as he is a canadian, and all they need is his passport to run background checks on him.

nope, the option c is separate from the tax return. that was our mistake when my husband did his taxes. it is a form that he can only get by calling CRA. again, the online version will not be accepted. He only needs the last year he's done his taxes for.

most likely they will not return the application, they will send a letter requesting the missing document, and give you a certain number of days to send it before returning the application. CIC will let you know if the application is not considered complete if the document is not acceptable.

i've heard on this forum that the online print out is not acceptable. CIC specifically requests the original version. so you'll have to tell us if CIC accepts the online version. It'll make the process easier for others! If you get an AOR and SA, you are good to go. They will send out the form requesting the document before then.

Definitely no need for the PR card form. The first PR card is sent automatically after the new PR lands. No application needed.

It takes 10-12 days to get the form? Crap.. My FBI clearance expires 27th.. If I can't get it before that what do you recommend I do?? Include 12 months of pay stubs?? How hard is it to get the notice of assessment?? How many days does that take? I'm starting to panic a bit.
