About PPR self-addressed envelop - "From" Section

I got the PPR email from CIC. In the self-addressed envelope, I put my address into "TO" section. But how to write the "From" section (including Date, Tel, and Address)?

They (CIC) are going to put a sticker on it
it comes like:

Hello guys is correct the address for send the passport. Because I received email ready for visa with this address
