I saw people starting their process with PCC. (I am an Indian passport Holder).

First, apply PCC at https://www.blsindia-canada.com/
step1 : go to this site https://embassy.passportindia.gov.in/ and fill the form PCC and take a print. After printing out affix a photo. In this form, you need to do two signatures.
step 2 : collect below documents
2.1 print checklist https://www.blsindia-canada.com/CanadaVisaCCT/toronto-PCC-checklist-rev-10.6-FEB-05-2020.pdf ( in checklist two signatures)
2.2. for fee visit https://www.blsindia-canada.com/pol_clr_cert.php.
for one person required $44.40 draft favour of BLS International Services Canada Inc.
2.3. For Indian Passport holders, please bring copies of first and last page, copies of all immigration stamp pages to India, (photocopy)
2.4 status in Canada ( work permit ) (photocopy)
2.5 Valid Driver’s license or photo ID issued by the Ontario Government, or any utility bill (less than 6 months old)(No Bank statements accepted) (photocopy )

step 3: once you ready step 1 and step 2 pay courier charges in this web site, https://blsindia-canada.com/vas/ . you need to pay for pick and drop off both.

Note: do not send an original document for your PCC. only Draft and step 1 form and checklist should be original.
 
The Further information is required to support that your business had a minimum of $500,000 in total gross annual revenue in its most recently completed fiscal year.
o Please provide:
• Canada Revenue Agency Schedules 125 (Income Statement Information) and 141 (Accounting Practitioner Information) for the most recently completed fiscal year prior to the date of application submission.
OR
• The financial statement for the most recently completed fiscal year, prior to the date of application submission signed by a chartered professional accountant (CPA).
You have 15 calendar days from the date of this letter to provide the additional information and/or documentation to the OINP by email at: ontarionominee@ontario.ca
If you have questions, please contact the OINP at the above email address.
Should the program not receive the requested additional information within 15 calendar days, your application will be assessed based on the information already provided.

Don't we need to do an upfront medical for the paper based application? or will we be getting a medical request later?
We can do upfront medical, but medical result which has been effective in a year should not be expired before decision made. If expired, redo and additional cost. If you want to do upfront medical, you have to see other's timeline. As I know, applicants whose AORs were June and July have not been requested medical yet.

How long is PCC valid for? I got one in October 2019 when visited back home, so can I still use to apply in this year? I was thinking it is valid for 1 year, so I can use it if I apply before October 2020...right or no?

You are right it is valid for one year.
