Well i am afraid I cannot provide you any sample because it was not a requirement for my case. The WES form is simpler. In that the first part with the heading "For applicants" is to be filled by you.

No courier this form to the relevant department of your UK institute after filling and signing it. You may keep a scanned image for your own reference. And first drop a mail to them explaining them your situation and follow up on their response. If they are ok with a scanned copy only then do it. Otherwise do not take such risk.

Well I think that along with the "transcript request form".... you should include a cover letter explaining your situation and copies of documents that prove that you have been a student there for specified time period (e.g. admission letter, any student id document etc.). Thay may not understand... so you have to explain in cover letter.
Transcript is a standard document of any institute... I dont think they would be able to write anything that you request. I dont have any notes... Im sorry.

In cover letter you could mention that you have been a student there for so n so period and that you are in process of applying for Immigration to Canada. For this you need them to send attested transcripts to WES as part of educational credential assessment. So kindly dispatch an attested copy of transcript in a sealed envelope as per instruction of the "WES Transcript Request Form"..... Hope you get the idea.

Thanks and I got replied from the my UK university about my Transcript..

http://www.wes.org/ca/required/International_Transcript_Request.pdf .They are asking that , Should they have to attach this form with Transcript and put in Sealed Envelope? ..OR this form is only for requesting Transcript from Applicant to their University ?

The Academic Transcript Request forms are tools that you can use to obtain the documents you need, however it is not a requirement to have the Academic Transcript Request forms completed. what this underline sentence stand for??


The attestations are only for certain countries, please follow the country specific required document website exactly - sealed and stamped envelopes are the requirement for all countries.

ECA is not required for Canadian degrees so you do not have to get it.

Please do not get confused by this statement. It simply means that this form is not required to be filled as part of the package that you send from your side to WES. It is just to facilitate you in getting the documents from your institute.

In such case perhaps you can skip the step of sending request form to the university but do convey your WES Application reference no. to them so they can quote it when sending your transcripts to WES. This will make things easier for WES to match your package with the sealed envelope from your university and avoid delay in processing.

To be on safe side, at least email a scanned image of transcript request form (your part filled and signed) to your university and request them to fill and stamp it if possible.
