Well i am afraid I cannot provide you any sample because it was not a requirement for my case. The WES form is simpler. In that the first part with the heading "For applicants" is to be filled by you.
No courier this form to the relevant department of your UK institute after filling and signing it. You may keep a scanned image for your own reference. And first drop a mail to them explaining them your situation and follow up on their response. If they are ok with a scanned copy only then do it. Otherwise do not take such risk.
Well I think that along with the "transcript request form".... you should include a cover letter explaining your situation and copies of documents that prove that you have been a student there for specified time period (e.g. admission letter, any student id document etc.). Thay may not understand... so you have to explain in cover letter.
Transcript is a standard document of any institute... I dont think they would be able to write anything that you request. I dont have any notes... Im sorry.
In cover letter you could mention that you have been a student there for so n so period and that you are in process of applying for Immigration to Canada. For this you need them to send attested transcripts to WES as part of educational credential assessment. So kindly dispatch an attested copy of transcript in a sealed envelope as per instruction of the "WES Transcript Request Form"..... Hope you get the idea.
