About CAIPS NOTES period

Now, I have 2 questions regarding the forms to be filled.

1. IMM5475E - It is mentioned in the form that you need to send the form to the office you submitted your original application. Do I need to send the same to CIO - Nova Scotia OR CHC - New Delhi
2. IMM5563B - In the first line under "Required Information" Embassy details is to be mentioned. Do I write CHC - New Delhi here?

2- ND embassy where u live

Someone told me that Consent Form is to be attached with IMM5563B, which my relative will be submitting in canada. I need not send the same either to ND or to Nova Scotia. What do you say?

All the docs from your side has to go to your relative and he will add his own Request letter and send it off to the Ottawa address. Nothing has to be sent to ND or Nova Scotia......CAIPS is an entirely separate organization ...it has no connection with CIC/or any other VO.

So here's the final check list:
1.http://www.cic.gc.ca/english/pdf/kits/forms/imm5476e.pdf ..... Fill this form (2 separate, by u and spouse), date and sign
2. Two consent letters - From you and spouse (Chk the attachments above)
3. IMM5653E - Provide your details in Section A and B (It's better if you fill out your details in a fresh form and take a print) - Send the scanned version to relative and relative will fill section C and D.
Leave the REQUESTER"s Reference no. as BLANK

4. Formal Request Letter - From Relative
5. Proof of residency – Relative will attach it
6. Fee payment – Relative will do the needful (attach the CC form)
