A lot of errors in medical report form from Singapore...

Don’t go with these errors. Though it will delay your file but it is always good to go after correction by VO.
Please contact with VO immediately raises your concern politely and ask them to resend the form after rectifies the errors in all areas.

I have never heard this typa whimsicality before from CHC Singapore.
They should have double checked, or I guess its the error of your consultant that followed all the way to Singapore. I'd so what sreja did. You can take help from Canadian high Commission in Dhaka, ask them what do they say about this.

Send a detailed e mail to your visa office and you will have their response in 2-3 days and then you can undertake medicals. Going to doctor you should take that mail and give him copy , the same copy he would send with his medical report to visa office. Your time would not be wasted on this account.
