A little help regarding filling of IMM5669 Form

Dear All,
Recently we are asked to fill and submit "Schedule A: Family Background/Declaration Form" by the Canadian Immigration. In this form there is a box on top right corner that says "Client ID/UCI/FOSS ID" and the caption above that box reads "FOR CIC USE ONLY" .
My confusion is that, do i have to type in my UCI in that box or not?

hello do they return your application? what are the comments pf CIC regarding your IMM669? are you inland application?

usually the box on the upper right corner will be for VO. no need to write anything there

This form is requested by Canadian Immigration to be filled by My mother, me and my father and submitted within 45 days.

I am also asked to fill the IMM5669 form after submission of PR Application myCIC. I think your question is valid since we already have UCI Number. But, it is outrightly written on the box that "FOR CIC USE ONLY" So I am not sure if we can fill in that box or not.

Did they ask to fill in this form for your dependents also ? IN my case it is only for me (not for my spouse.)

Actually my mother is the principal applicant and i am the dependent child while my father is spouse. We are asked to fill this form independently and sign it. I think we dont have to write our UCI ID but in the covering letter we have to mention our UCI and also send the letter that we received along with these documents..

d) Has XXXXXX been refused refugee status, or an immigrant or permanent resident visa (including a Certificat de sélection du Québec (CSQ) or application to the Provincial nominee Program) or visitor or temporary resident visa, to Canada or any other country?-
I checked YES because I have been refused for open work permit

e) In Schedule A, it has been asked if I have ever been refused admission to, or ordered to leave Canada or any other country?:
Should i say yes here as well since I have never been refused admission unless it means visa denial.

1. No you dont fill anything that for immigration office use
2d . You selected the right option and i guess you need to explanation about it
2e. You select No, because this is regarding denial not refused; denial can mostly based on fraudulent information/crimination history , meaning mislead the immigration office where the visa was cancelled make the applicant non admissible to Canada

You can wait for other to respond, if you want to get second opinion on ans 2e

For question 2e, is it not same as the statutory question in the actual Express entry application? Because there i answered to both questions as Yes.
Schedule A is regarding principal applicant , not sure you meant , you applied express entry and checked yes . If that worked for you try to provide the details for it for principal applicant aswell, with the same answer as 2d .

Yes @ramu_rokey , in ee profile there are two questions which one of it is exactly same with question d and the other one pretty similar to e section with slight difference, and since me as principal applicant and my husband as spouse both have visa refusals in the past we answered yes in ee profile, but here in schedule A form the term “admission “ is what making the confusion. Some says this means denial of entry at the border and it’s not visa refusal.

I don't know precisely which part of the form you are referring to but IRCC basically considers visa refusals and denials of entry to be the same / both things that MUST be disclosed.
