The part where it says The Marriage between .. Should I put and between the 2 names?

Number 3. I intend to marry ... in the hotel. I don't know the exactlocation .. can I just put in the [city] , province of [province], [Country], or I have to put in my spouse's address

and again Number 3. In my case, I don't know the exact date, then I have to put during the week of June 1st 2013 to June 8th 2013?


and on the last part Declared before me at .. .. and on the the right hand side of it, it says full name.. it it where the lawyer has to write his name down?

It is a solemn affidavit to state that you are single, and you are getting married. So you should decide with your spouse about the unknown place and date before preparing the form. However some answers:
