A big help for a small question

Hi Everyone,
I wish to thank in advance for taking time to read this post.

My grandpa wants to apply for visitor visa to visit me in canada.

His name in Passport is eg: Johndavid smith
His name in relationship document and bank and property is eg: John David smith

Does this require a affidavit or just writing two names in question 2 of IMM5257 (have you used any other names)

he also does not know how to do signature, can he just put ink-thumbprint or "X' or NA or representative's signature or leave blank?

The "used other names" is enough as it is obvious he is the same person (this happens to other people in non-English America for example).

For the other issue, he can't read/write? He can put his name, that's enough. If he is unable to do that then appointing a representative would be the best choice.

Thank you very very much. Sorry if i misunderstood, if he cannot read and write, his representative can write his name in the signature panel, right? does the representative have to do initials. I searched for this topic everywhere on the web but could not find anything related. I have asked the visa office but have not yet received any response.

Sorry my bad. I was aware of the use of representative but i was not aware that the representative could write applicant's name in the signature panel. thank you again for the link
