Hi,

There seems to be many posts about this form and topic but none that seems to answer my question, but it seem like such a very common one.

On the form we must provide names in English and native language. That I fully understand.

For my wife (Applicant), her mother and father were born in China and naturally have had their names in written Chinese characters on their documents and such. But for herself and her siblings, they were born in an English speaking country so their names, even though are Chinese names, were written in English only pronunciation on their birth certificates. Aside from use at home within the family, the Chinese characters of their names would not have been on any official documents like passports.

In a situation like this, the only names to provide on form 5406 for my wife and her siblings would be the English versions of their names, right? Even though they had been given "Chinese" names by their parents.

Thanks
