I'm applying for a citizenship certificate for my baby. Requirements state I must submit "Two (2) pieces of personal identification, such as a passport, a driver’s licence and a health insurance card, one of which must have your photo on it". It also states that "Social Insurance Number (SIN) cards, bank cards and credit cards are not acceptable pieces of identification".

Other than his passport, I have no other identification for him… He's 18 months old!!

Can readers please share what you used as the second identification document?

THANK YOU.

Re: 2x identification documents... For a baby.

Passport and health card should do. Province like ON will have a photo on the health card but Alberta for example does not have the picture. But you only need one with a photo so the passport should meet that req. health card will be the second one.

No pictures on health card for kids in Ontario, if that's what u meant yes passport and health card makes 2 pieces of I'd

Re: 2x identification documents... For a baby.

We are in America. His health care card only has his dad's name on it.

Can we use the birth certificate as a second form of ID? Because the birth certificate is already required in the package.

Re: 2x identification documents... For a baby.

Provincial health card issued in Ontario has the name of the child. American health insurance card may not be proper I'd if that's what u meant. Passport and immunization record might be an option...

Re: 2x identification documents... For a baby.

Immunization record. Maybe I'll submit that, along with a letter explaining that he's 18months and doesn't have any ID since in the US his health care card only has his dad's name on it.

If anyone else has any input, please let me know.

Thanks all.

Re: 2x identification documents... For a baby.

The instructions say that passport copies and hospital/ immunization record for non school-age children as id proofs.
so you should be fine...

Re: 2x identification documents... For a baby.

birth certificate is needed anyway, so passport, immunization + birth certificate
