I have passed 2015 parents immigration sponsorship stage

My parents have not received medical checkup request.

What if I am getting married now? Do I have to get my sponsorship re-approved? What's the exact process?

you will need to inform CIC of the change , pay your wife's fees and do medicals etc , it will take time . if you don't declare her you can't sponsor her ever and you could lose your PR for misrepresentation .

The wife is Canadian citizen as well.

Your family size will increase by +1 in the year you get married. So starting the tax year you get married, your LICO income requirement will increase a bit. If needed you can add your wife as a co-signer to add her income to yours if needed to stay qualified.
